In recent decades, there has been a paradigm shift in understanding the relationship between obesity and cardiovascular risk. Epicardial fat, as a metabolically active paracrine organ, is considered a potential independent predictor of cardiovascular pathology.</p> <p>The purpose of this review is to systematize current data on the relationship between epicardial fat volume and cardiovascular risk, with a focus on the pathophysiological mechanisms, quantification methods, and clinical significance of this indicator.</p> <p>Material and methods. This literature review was performed using a systematic approach to search, selection and analysis of scientific publications devoted to the relationship between epicardial fat and cardiovascular risk. The information search strategy included the use of electronic databases of medical literature: MedLine, PubMed, CochraneLibrary, nih.gov and e-library by keywords for the period from 2003 to May 2025.</p> <p>Results. The paper analyzes methodological approaches to the quantitative assessment of epicardial fat and systematizes data on its correlation with traditional risk factors, indicators of inflammation, and insulin resistance. The paper also examines the pathophysiological mechanisms that link excessive accumulation of epicardial fat to the development of atherosclerosis and other cardiovascular pathologies. Special attention is given to the threshold values of epicardial fat volume, exceeding which significantly increases the risk of cardiovascular complications. The article presents data on the prognostic value of quantitative assessment of epicardial fat and the possibility of its inclusion in algorithms for stratifying cardiovascular risk.</p> <p>Conclusion. The combination of the studies reviewed indicates that epicardial fat may serve as a valuable biomarker of cardiovascular risk and a potential target for therapeutic interventions.</p></abstract><trans-abstract xml:lang="ru"><p>Обоснование. В последние десятилетия произошла трансформация парадигмы в понимании связи между ожирением и сердечно-сосудистым риском. Эпикардиальный жир, являясь метаболически активным паракринным органом, рассматривается как потенциальный независимый предиктор кардиоваскулярной патологии.</p> <p>Целью настоящего обзора является систематизация современных данных о взаимосвязи между объемом эпикардиального жира и сердечно-сосудистым риском, с акцентом на патофизиологические механизмы, методы количественной оценки и клиническую значимость данного показателя.</p> <p>Материал и методы. Настоящий обзор литературы был выполнен с использованием систематического подхода к поиску, отбору и анализу научных публикаций, посвященных взаимосвязи эпикардиального жира и сердечно-сосудистого риска. Стратегия информационного поиска включала использование электронных баз данных медицинской литературы: MedLine, PubMed, CochraneLibrary, nih.gov и e-library по ключевым словам за период с 2003 года по май 2025 года.</p> <p>Результаты. Проанализированы методологические подходы к количественной оценке эпикардиального жира, систематизированы данные о его корреляции с традиционными факторами риска, показателями воспаления и инсулинорезистентности. Рассмотрены патофизиологические механизмы, связывающие избыточное накопление эпикардиального жира с развитием атеросклероза и других сердечно-сосудистых патологий. Особое внимание уделено пороговым значениям объема эпикардиального жира, при превышении которых значительно возрастает риск сердечно-сосудистых осложнений. Представлены данные о прогностической ценности количественной оценки эпикардиального жира и возможности ее включения в алгоритмы стратификации кардиоваскулярного риска.</p> <p>Заключение.
