The influence of temperament type on blood pressure desynchronosis in residents of the Northern Region

Keywords: North, desynchronosis, circadian rhythm, ultradian rhythm, blood pressure, arterial hypertension

Abstract

Background. Arterial hypertension (AH) is one of the most common cardiovascular diseases. Its prevalence in Russia has reached 53.9%. Exposure to the unfavorable factors of the North leads to disruptions in circadian rhythms, including blood pressure. Many authors consider this a sign of the risk of developing AH.

Purpose. To study the structure of blood pressure rhythms in residents of the northern region of Russia (Khanty-Mansiysk) with arterial hypertension depending on temperament type.

Materials and methods. All study participants had their blood pressure assessed using the generally accepted ABPM method. Higher nervous activity type was assessed using the Eysenck EPI questionnaire modified by A.G. Shmelev. Based on the test results, each participant was classified into one of the following temperament types: melancholic, sanguine, choleric, or phlegmatic. The "Wavelet Analysis of Biological Rhythms" computer program was used to detect circadian and ultradian rhythms, as well as desynchronosis. To assess blood pressure rhythms depending on temperament type, the total sample was divided into a group without hypertension and a group with hypertension. Kruskal-Wallis analysis of variance was used to identify differences in blood pressure parameters between individuals with different temperaments.

Results. In the group of individuals without hypertension, choleric individuals had statistically significantly higher nighttime SBP and nighttime DBP values compared to melancholic individuals. Cholerics also had statistically significantly higher nighttime DBP values compared to phlegmatic individuals. In the group of individuals with hypertension, sanguine individuals had statistically significantly higher daytime DBP values compared to melancholic individuals. In the group without hypertension, individuals with both choleric and phlegmatic temperaments exhibited circadian rhythms: SBP and DBP in choleric individuals, and SBP, DBP, and HR in phlegmatic individuals. Sanguine individuals exhibited ultradian SBP and DBP rhythms, as well as disorganized HR rhythms. Melancholic individuals exhibited disorganized SBP, DBP, and HR rhythms, indicating desynchronosis. In the group with hypertension, sanguine individuals exhibited circadian rhythms of SBP, DBP, and HR. Phlegmatic people have one circadian rhythm – systolic blood pressure (SBP), while choleric people have two ultradian rhythms – diastolic blood pressure (DBP) and heart rate (HR). Melancholic people exhibit disorganization of the SBP and DBP rhythms and a circadian rhythm of heart rate.

Conclusion. When diagnosing hypertension and assessing the effectiveness of antihypertensive chronotherapy, it is important to consider the type of higher nervous activity.
